Kennametal Compression - Tension Tap Collet Chuck # SS100TCK719, 44484CK3.
It seems to function fine with a hand check.  The spring loaded part has about 1/4" of smooth in and out tension.  We believe this uses a "K" series collet.  NO collets included.  The top nut is worn quite a bit (last 3 photos) and still functions fine and a "flat" has been ground in the shank.  It is 7" OAL and the collet body should be around .680".
